After the discontinuation of Step 2 CS in January 2021, the Educational Commission for Foreign Medical Graduates (ECFMG) introduced six alternative Pathways to meet the clinical-communication skills requirement.

What is the differences between the pathways
Pathway 1
Hold or have held an unrestricted medical license in your country
Pathway 2
Pass an OSCE required by your medical school
Pathway 3-5
Graduate from a program accredited by WFME, NCFMEA, or partnered with a U.S. medical school
Pathway 6
Complete Mini-CEX evaluations by licensed physicians—especially for those who’ve failed Step 2 CS

Why They Matter for The Match
An ECFMG certificate is required to enter ACGME-accredited training and to apply for USMLE Step 3. ECFMG certification is mandatory for the 2026 NRMP Match.
While you can technically apply to ERAS in September without certification, most U.S. programs expect it. Having completed a Pathway by ERAS gives you a competitive edge and avoids delays.
Why Pathway (1) Is Most Popular Among Egyptian Applicants
Egyptian graduates frequently already hold a medical license and a good-standing letter from the Syndicate. This means that pathway 1 is both the simplest and fastest route for Egyptian IMGs, provided the applicant is a graduate with a medical license.
Important Information for Egyptian Applicants
Even if your license has expired, you can head to the Egyptian Medical Syndicate and renew it. You will also need to ask them to approve your ECFMG application by sending a confirmation email including your Letter of Good Standing directly to ECFMG. This way your application will be ready for review if you have all other requirements (Steps 1 and 2, OET).
Required Documents & Timing
Each pathway requires documentation such as an unrestricted medical license (Pathway1), an OSCE or attestation letter (Pathway2), school accreditation evidence (Pathways 3-5), or Mini-CEX evaluations (Pathway6). All applicants must provide OET scores.
After submitting, ECFMG usually processes paperwork in a few days to weeks, with certification issued once everything is verified.
Tips for Egyptian Applicants
Plan effectively:
- Begin gathering licenses and good-standing letters in advance.
- Register early for OET—aim to have results by December 2025.
Keep an eye on ECFMG updates; applications typically open June/July.
